FREQUENTLY ASKED QUESTIONS

Do I have to submit all the supporting documents including my health history, which are mentioned in the application form?
Yes, applicants have to provide all relevant medical records and/or check-up reports which are material for the underwriting process. We may check with any hospital, physician, insurance company or organisation that has any records or knowledge of applicant or applicant’s health upon applicant’s authorisation.

I had certain medical impairment but I forgot to disclose on my application form.  What should I do?
If any material fact relating to health related information of the Insured Person which may impact the risk assessment by the Company is incorrectly stated in, or omitted from, any required document submitted to the Company for the purpose of the application, the Company has the right to declare this Policy void as from the Policy Effective Date and notify the Policy Holder that no cover shall be provided.

If you have certain medical impairment omitted from your application, you need to provide such health related information to the Company immediately.  The Company would re-underwrite the terms and benefit based on the updated information.  Premium Loading, Case-based Exclusion(s), or rejection of the application may be applied as a result of the underwriting.

What is Premium Loading and Case-based Exclusion?
"Case-based Exclusion" shall mean the exclusion of a particular Sickness or Disease from the coverage of these Terms and Benefits that may be applied by the Company based on a Pre-existing Condition or factors affecting the insurability of the Insured Person.
